This course is designed to give strategic decision makers, in healthcare, an overview of the philosophical and methodological principles which sit behind modern healthcare safety investigations. Please note this course if only for executive and non executive director level only.
Participants will be guided around the domains which drive the HSSIB investigation science curriculum so that they have information about what to expect from investigations in the future.

This course offers an overview of the principles that underpin a professional safety investigation interview with either a member of staff, a patient or a family. It is designed to enable those undertaking investigative interviews do so in a structured and supportive manner, which is of vital importance to the success of PSIRF. The course includes:
Planning and preparing for an interview
Using a structured hierarchy of questions to facilitate comprehensive, accurate information
Asking system-focused questions
Closing an interview
The course aligns to the PSIRF guidance on a systems approach to interviews. The course takes place as a 3.5 hour live virtual workshop and includes interactive activities and discussions. You will also be directed to specific activities designed to extend your knowledge.

This course is designed to support report writing for investigations and other learning responses. A written report is the main way healthcare organisations communicate their findings and actions taken so it is extremely important to make that learning document impactful and accessible. The course will show you how to achieve that and includes:
Important elements of report language including how to write in plain English and using inclusive, neutral language
The common pitfalls when writing reports following investigations and other learning responses
The application of report language to the PSIRF investigation report template
An introduction to a tool to help support the review and development of learning responses
Factors to consider when reviewing investigation reports and other learning responses
The course takes place as a 4 hour live virtual workshop.
This course is aimed at those who are planning to use, or may already be using, After Action Review as one of their learning responses to patient safety events. It will also be useful for those in Patient Safety Incident Response Framework (PSIRF) oversight roles. The course includes:
Defining what After Action Review is
Understanding the principles of After Action Review
Discussing the attributes needed to be an After Action Review facilitator
Exploring how to conduct an After Action Review
Reflecting on the value of After Action Review

This course takes place as a 3.5 hour live virtual workshop
This course will introduce learners to the approach of thematic analysis for safety investigations. Thematic Analysis allow the identification of patterns across organisational data to generate insights on safety themes and issues. The focus of the course is to help learners understand how thematic analysis can be applied within their own organisations, what factors they might consider in using the approach and some of the challenges in using it as part of the broader patient safety strategy.
Oure aim is that by demystifying this form of analysis it will allow you to reflect on how you may have already used this approach to some extent, but also how you could expand or introduce this approach to enhance organisational learning.
