This CIEHF accredited course is aimed at those who lead investigations and other learning responses and those in Patient Safety Incident Response Framework (PSIRF) oversight roles. The course includes:
An introduction to complex systems, systems thinking and human factors
Investigation practice such as interviewing, capturing work as done, use of a systems framework (SEIPS), data synthesis, and report writing
Developing effective safety actions and recommendations
Engaging and involving those affected by patient safety incidents
This is an on demand, online, learning course with bitesize modules for you to learn at your own pace. The course takes 20 hours, on average, to complete and you are awarded 20 CPD points.

This one-day live course is aimed at those who have a lead role in engaging and involving those affected by patient safety incidents.
The day is split into three modules:
Creating the right foundations – this module will explain the organisational conditions that need to exist for meaningful involvement to flourish.
Before and initial contact – this module will discuss the importance of first contact and explore what makes communication effective, and the options that exist when engagement breaks down.
Continued and closing contact – this module will consider the challenges of ongoing contacts and discuss how best to manage the sharing of learning responses. It will also include an appreciation of the health and wellbeing of engagement leads.
This highly interactive course will involve presentations, group activities and discussions. Please be aware that to reach the PSIRF minimum training requirement you need to attend the whole day and are awarded 7 CPD points.

This course is designed to introduce the principles of patient safety incident response oversight. This course covers the mindset and the culture underpinning effective oversight, as well as the theory and practice of measurement and monitoring of patient safety. Students will also have an opportunity to explore thematic analysis as a method for organisational learning across organisational boundaries. . This course is run in a single day, 0900 - 1600. You are awarded 7 CPD points.
